Indications/Uses
HTN & CV risk reduction in patients unable to take ACE inhibitors.
Dosage/Direction for Use
Essential HTN 40 mg once daily or 20 mg. Max: 80 mg once daily. CV prevention 80 mg once daily. Severe renal impairment or haemodialysis Starting dose: 20 mg. Mild to moderate hepatic impairment Max: 40 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Biliary obstructive disorders. Severe hepatic impairment. 2nd & 3rd trimesters of pregnancy.
Special Precautions
Patients w/ cholestasis, biliary obstructive disorders or severe hepatic/renal impairment. Renovascular HTN. Intravascular hypovolaemia; symptomatic hypotension may occur in patients who are vol &/or Na depleted by vigorous diuretic therapy, dietary salt restriction, diarrhoea, or vomiting. Inhibition renin-angiotensin-aldosterone system, hypotension, syncope, hyperkalaemia & changes in renal function (including acute renal failure). Not recommended in patients w/ controlled BP & should closely monitor renal function. Acute hypotension, hyperazotaemia, oliguria, or rarely acute renal failure in patients whose vascular tone & renal function depend on activity of the renin-angiotensin-aldosterone system (eg, patients w/ severe CHF or underlying renal disease, including renal artery stenosis). Not recommended in patients w/ primary aldosteronism. Patients suffering from aortic or mitral stenosis, or obstructive hypertrophic cardiomyopathy. Hyperkalaemia. Fructose intolerance. Less effective in lowering BP in Black people. MI or stroke. Dehydrated patients. Pregnancy. Childn (<18 yr). Elderly w/ compromised renal function.
Adverse Reactions
Hyperkalaemia; syncope; hypotension, orthostatic hypotension; acute renal failure. Sepsis; anaphylactic reaction.
Drug Interactions
Risk of hyperkalaemia w/ salt substitutes containing K, K-sparing diuretics, ACE inhibitors, angiotensin II receptor antagonists, NSAIDs, including selective COX-2 inhibitors, heparin, immunosuppressives (cyclosporin or tacrolimus) & trimethoprim. Increases in serum lithium conc & toxicity. May reduce antihypertensive effect of COX-2 inhibitors & non-selective NSAIDs. Increase in AUC0-24 & Cmax of ramipril & ramiprilat. Vol depletion w/ high dose diuretics eg, furosemide (loop) & hydrochlorothiazide (thiazide). Potentiate hypotensive effects w/ baclofen, amifostine. Orthostatic hypotension may be aggravated by alcohol, barbiturates, narcotics, or antidepressants. Reduction of antihypertensive effect w/ corticosteroids (systemic route).
